Mass Vaccination

In the event that a communicable disease outbreak or mass exposure to a dangerous biological substance occurs in CrawfordCounty that requires an immediate response to vaccinate the public, the local health departments have developed a joint response plan that facilitates the establishment and operation of a mass vaccination and/or medication dispensing clinic in CrawfordCounty.

Example 1: If a Smallpox outbreak occurs, local, state and federal officials would be notified. If the decision is made to vaccinate the public, a mass vaccination clinic would be established in Crawford County.

Example 2: If a large number of county residents were exposed to Anthrax, local, state and federal officials would be notified. If the decision is made to dispense medication to those exposed, a mass dispensing clinic may be established in Crawford County to quickly dispense antibiotics.

In order to treat the largest number of people in the smallest amount of time for maximum treatment effectiveness, it is important to reduce the amount of time it takes to process an individual from start to finish at the clinic. Therefore, we have provided the following forms that can be printed and completed prior to your arrival at the clinic. If there are any questions on these forms that you are not comfortable answering, then leave that specific question blank and continue to complete as much of the form as possible. Upon your arrival at the clinic, you will have an opportunity to discuss your questions with a medical professional prior to treatment.

Smallpox Outbreak Only: Go to the following website, print, read and complete the forms prior to your arrival at the clinic. Wear a sleeveless or short sleeve shirt to the clinic as privacy panels will not be used in a mass vaccination setting.

Anthrax Exposure Only: Complete this form and bring it with you to the medication dispensing site along with your photo ID. One adult family member may pick up medication for up to twelve other immediate family members as long as their names and information are included on the form. Sending only one adult family member to the clinic will help us dispense medication at a much quicker pace.

Mass Medication Dispensing:  Use the same form and instructions as for Anthrax Exposure above.

Mass Vaccination (not smallpox):  Complete this form and bring it with you to the vaccination clinic.  Wear a sleeveless or short sleeve shirt to the clinic as privacy panels will not be used in a mass vaccination setting.
